Agent
Person who is authorized to make decisions on behalf of another person.  The authority to make decisions comes from having a properly executed Health Care Proxy and Durable Power of Attorney.
Alternate Health Care Proxy
Also known as alternate health care agent, is the person the principal chooses to make healthcare and other personal decisions on behalf of the principal when the principal lacks the capacity to make those decisions and the nominated primary health care agent is unable or unwilling to serve as health care agent.
Capacity
Ability of the principal to understand and make personal and/or financial decisions. 
Codicil
 an addition or supplement that explains, modifies, or revokes a will or a part of one.
Conservation
Person authorized by the court to make financial decisions and other decisions in regards to property on behalf of an incapacitated person or minor.
Legal document whereby a principal designates a person (agent or attorney in fact) to make financial and other decisions related to assets.  Durable means that the authority continues even when the principal lacks capacity.
Guardian
The person authorized by the court to make health care and/or personal decisions on behalf of an incapacitated person or minor.
Legal document whereby a principal designates a person (agent or proxy) to act in his/her place when the principal lacks the ability to make health care and other personal decisions.  Depending upon the state in which you reside, it is also known as Durable Power of Attorney for Health Care; or Power of Attorney for Health Care.  Health Care proxy or Health Care agent also refers to the person who makes decisions on behalf of the principal.
Intestate
A legal term for when one dies without a Will or estate plan.
Principal
The person who is assigning someone else to act as his or her agent for such things as health care and financial decisions.
